In the year between graduating college and entering medical school, Dr. Smith studied public health through the UT School of Public Health. She then went to medical school at The University of Texas Medical Branch in Galveston. At UTMB, she completed the Bilingual Health Track and participated in integrative medicine and nutritional counseling clubs.
After graduating from medical school, Dr. Smith moved to San Antonio to train at the CHRISTUS Santa Rosa Family Medicine Residency program. She was able to take care of a wide variety of patients in both the clinic and hospital settings.
Dr. Smith continues to serve the San Antonio community. In her free time, she likes to hike, lift weights and spend time with her significant other and their two dogs and cat.

ANGELINA IYINBOR, MD

Dr. Iyinbor grew up in San Antonio and is excited to be returning home to care for her community. She is passionate about helping her patients reach their health goals by working with them as a team to focus on what truly matters to them. She aims to foster a relationship with her patients that goes beyond their medical diagnoses.
Dr. Iyinbor earned her Bachelor of Arts degree in Biochemistry and Cell Biology from Rice University in Houston, Texas, and received her medical degree from UT Southwestern Medical Center in Dallas, Texas. She then left Texas for the East Coast where she completed her internal medicine residency training at Yale New Haven Hospital in Connecticut. Throughout residency, Dr. Iyinbor became proficient in a variety of outpatient procedures, including knee, carpal tunnel, and trigger finger injections. She is also working towards obtaining her certification in Lifestyle Medicine and is excited to provide holistic care to her patients. Her clinical interests include lifestyle medicine, culinary medicine, preventative care, outpatient procedures, and chronic disease management, particularly diabetes
and hypertension.
In her spare time, Dr. Iyinbor is an avid ballroom dancer and has a wide range of outdoor hobbies including snowboarding, rock climbing, kayaking, stand up paddle boarding, and hiking. She also prioritizes her emotional wellbeing through meditation, journaling, and spending time with friends and family.

MARIJAN DE LA FUENTE GILLARD, MD

Dr. Gillard was born and raised in San Antonio, Texas. She moved to Houston for medical school and later returned to the UTHealth-San Antonio, where she completed her residency in Family Medicine in 2002. Recently, Dr. Gillard celebrated her twenty-fifth graduation anniversary from the McGovern Medical School at UTHealth-Houston.
Her career in Family Medicine began in private practice, after which she transitioned to a hospitalist role for patients admitted to subacute or rehabilitation facilities in San Antonio. She then joined the teaching faculty at UTHealth-San Antonio, eventually becoming the Director of Primary Care at the Medical Arts and Research Center. Dr. Gillard also served as the medical director at Baptist Health System, where she pioneered their primary care initiative. In 2015, she shifted her focus to Wellness medicine, joining Health by Design as a Wellness Physician. Passionate about preventive care, Dr. Gillard is currently working toward her certification in obesity medicine. She looks forward to supporting the health of police and firefighter retirees and their families, inspired by her brother-in-law, a retired firefighter, and her nephew, who serves in the San Antonio Fire Department.
Outside of work, Dr. Gillard cherishes time with her husband—her high school sweetheart—their sons, and their two dogs. Together, they root for the Longhorns, Aggies, Texans, and Chiefs. Dr. Gillard also enjoys staying active in the Peloton community.

JASON HERNANDEZ, PT, DPT

My treatment philosophy is built on truly listening to my patients and fostering collaboration with their families and medical teams. For me, it all starts with a warm smile and an open mindset. I take the time to understand each patient’s unique needs and concerns, followed by a thorough examination and review of findings. From there, I focus on providing personalized treatment options because I firmly believe that no two treatment plans should ever be “cookie-cutter” approaches.
Dr. Hernandez is a dedicated and highly skilled physical therapy professional who completed his Doctor of Physical Therapy in 2012 at Texas State University San Marcos and obtained certifications in Manual Therapy and the Executive Program in Private Practice Management through Evidence in Motion.
Dr. Hernandez began his professional journey as a Physical Therapist (PT) in the Selma/Schertz area, where his dedication and expertise earned him a promotion to Clinical Director at his previous company. This role brought him to the Northwest side of San Antonio, where he has since focused on private practice in an outpatient setting. He emphasizes exceptional customer service and bedside manner, which he feels are often overlooked in modern healthcare. Believing in open and honest communication with his patients, Dr. Hernandez strives to foster trust and provide the highest level of care.
Outside of his career, Dr. Hernandez enjoys staying active through working out and deepening his spiritual growth by reading the Bible and devotionals. He cherishes time with his family, including his wife and two daughters, Norah and Catalina. His hobbies include cooking, watching football and other sports, and creating meaningful moments with loved ones.

Dr. Dominguez is Board Certified in Family Medicine and is a Fellow of the American Board of Family Medicine. From 1997 to 2006, she served as a Clinical Assistant Professor of Family Medicine at the University of Texas Health Science Center at San Antonio. She has been recognized as a “Best Doctor” in numerous publications, including US World News and Report, Texas Monthly and NSIDE Magazine.
In her free time, Dr. Dominguez enjoys spending time with her wonderful husband and their three children. She also enjoys dancing, horseback riding and traveling internationally to learn about new cultures, people and history.

Dr. Masozera is married to his wonderful wife, May, a mental health nurse. They have two children—a daughter in Graduate Pharmacy School and a son who is serving as an Officer in the US Air Force, alongside his wife, who is also an Air Force Officer.
He enjoys working with young people to help them realize their God-given talents for His glory. His hobbies include reading, listening to music, walking and playing table tennis.

Dr. Patteson is a Board-Certified Family Medicine physician in San Antonio, Texas and has served with Health By Design since 2014.
Dr. Patteson earned his undergraduate degree in psychology at Baylor University and is an AOA graduate of the University of Texas Health Science Center at San Antonio. He completed his Family Medicine residency at CHRISTUS Santa Rosa Hospital in San Antonio, Texas, where he served as chief resident of the program. After residency, Dr. Patteson remained on staff as a teaching faculty member for six years and served as the Associate Director for the residency program and the Director of the Family Health Center. Dr. Patteson is a third-generation family physician.
In his spare time, Dr. Patteson enjoys spending time with his wife and three children, hiking, and traveling.

Dr. Ramos has spent her professional career working with the underserved in community health centers. She worked first in the Rio Grande Valley, where she developed a program to improve outcomes for migrant farmworkers with diabetes. Then, for the past twenty-two years, Dr. Ramos has worked at a community health center in Boston’s Italian North End. Working as a primary care physician, she was affiliated with the Massachusetts General Hospital and taught Harvard Medical School graduates ambulatory medicine. In 2021,
Dr. Ramos returned to her roots in San Antonio, where most of her family resides.
She enjoys cooking, reading and studying astrology in her free time. In addition, she is a committed hot yoga student and enjoys weight lifting. She is proud to have placed first in two separate Strongman competitions.
Dr. Ramos loves meeting with all of her patients and is thrilled to serve in your Health by Design Wellness Center.

Dr. Schaffner and her husband Eric are happy to be settling in San Antonio and enjoy spending time with family, hiking and visiting museums. They have two children who bring them much joy, Kate and Luke. She is grateful to be part of Health by Design.

THOMAS CHILTON, MD

Dr. Chilton came to San Antonio for college, attending Trinity University where he earned a degree in biology with a minor in economics. After college, he spent several years working in the emergency room and falling in love with medicine. He went on to earn a master’s degree from UT Health Science Center in San Antonio before completing medical school as part of the inaugural class at the University of the Incarnate Word School of Osteopathic Medicine. Being deeply rooted in San Antonio, he chose to stay and complete a Family Medicine residency with the Texas Institute for Graduate Medical Education and Research.
Dr. Chilton strives to serve his patients as the complex human beings they are, supporting their total health journey and helping them become the best versions of themselves.
When not working, Dr. Chilton enjoys spending time with his wife and two children. He loves to cook, explore new restaurants, and travel. As an avid car enthusiast, he believes there is no better day than the right car on the right road followed by the right meal.

SAMANTHA HANSEN, PT, DPT, FAAOMPT

I believe pain is not a natural part of aging, and everyone has the potential to feel and move better. My goal is to empower my patients with the knowledge and tools they need to take control of their health. Through education, individualized exercises, and targeted mobility training, I help patients gain a deeper understanding of their bodies and how daily habits—like prolonged postures, repetitive movements, and nutrition—impact overall function. I believe that we are what we do repeatedly, so fostering healthy habits is essential for maintaining long-term well-being.
Dr. Hansen earned her Bachelor’s in Exercise Physiology from Texas A&M in 2008, followed by her Doctor of Physical Therapy (DPT) degree from the University of North Carolina in 2012. She has worked in outpatient orthopedics since then. Following graduation, she moved to the Bay Area of California, where she further enhanced her expertise by obtaining a Certificate in Applied Functional Science. In addition to clinical practice, she taught a weekly balance and strengthening class at a local senior center and treated athletes at Santa Clara University.
In 2016, Dr. Hansen returned to her hometown of San Antonio. Since then, she completed a three-year orthopedic fellowship and obtained her Titleist Performance Institute Golf Level 1 Certification. Her clinical expertise includes manual therapy techniques, movement system impairment-based intervention, and athletic injury prevention.
Outside of her professional life, Dr. Hansen enjoys spending quality time with her three young children and her dog at the park. She also loves exploring San Antonio’s vibrant breweries and restaurants. In her free time, she can be found reading, hiking, practicing yoga, or playing and watching sports—especially cheering for Aggie football.

HILDA GONZALEZ-SAENZ, MD

Dr. Gonzalez-Saenz is married (26 years) and has a daughter and a son. She volunteered at their parish teaching religious education to elementary school children and volunteers at her kids’ schools whenever possible. She enjoys working in her yard and gardening.

Rachelle Hamblin, MD
Roosevelt Wellness Center
Dr. Rachelle Hamblin is a board certified family medicine physician who is originally from Livermore, California but is now a Texan! She received her Bachelor of Science from UC Davis in California then moved to Texas to complete her Doctorate of Medicine (MD) and Masters of Public Health (MPH) at UT Health San Antonio Long School of Medicine. She then completed her Family Medicine Residency at CHRISTUS Westover Hills in San Antonio, Texas and is board certified by the American Board of Family Medicine.
She loves working with her San Antonio Fire Department (SAFD) and has been volunteering with them since 2019. As part of her passion to take care of her firefighters and extinguish firefighter cancer, Dr. Hamblin volunteers with the SAFD Cancer Committee; works on firefighter cancer prevention research with local, national, and international partners; teaches at the Fire Training Academy; advocates for occupational cancer prevention and treatment at local, state, and national levels; assists with cancer screenings; educates both physicians and firefighters on occupational cancer; and rides out with SAFD.
She has also completed the Citizen's Fire Training Academy. She has also recently begun working with the San Antonio Police Department to help them develop an occupational wellness program.
In her free time, she enjoys spending time with her partner, family, and friends; doing krav maga; and working out.

MELANIE FREDERICK, MD

Dr. Melanie Champagne Frederick is a Family Medicine physician who has 39 years of experience in patient-centered healthcare. With a lifelong passion for the art of medicine, she brings a diverse background in preventive and wellness care, geriatrics, and general adult medicine.
Dr. Frederick was born in a small Cajun town on the bayou in South Louisiana, the eldest of nine children. She went to LSU Baton Rouge, the home of the Tigers, where she was one of the first women in the ROTC detachment. She later served in the Air Force Medical Corps where she headed the Barksdale AFB Primary Care Clinic for several years. After her Air Force service, she practiced in Shreveport, Louisiana for 31 years and later moved to San Antonio to be closer to family. She has also worked at the VA servicing the needs of our veterans.
In her spare time, you will find Dr. Frederick cooking up some of her family’s favorite Cajun dishes – gumbo, jambalaya, and crawfish étouffée. She also loves hiking, nature photography, jewelry making, genealogy, community service, and gardening. Her all-time favorite is spending time with her young grandsons.

NICHOLAS BRANCH, PT, DPT

My approach to physical therapy is centered on treating the whole person to uncover and address the root cause of neuromusculoskeletal concerns. Through a combination of hands-on treatment and personalized guided exercise programs, I strive to empower every individual I work with to take control of their health and confidently manage their condition.
Dr. Branch obtained his Doctorate in Physical Therapy from George Washington University in Washington, D.C. and earned a board certification in Neurologic Physical Therapy in 2015. During his Active-Duty service, he also became a certified strength and conditioning specialist as well as a tactical strength and conditioning specialist.
Dr. Branch has over 16 years of experience practicing in both civilian and military environments, including time overseas. From 2019 to 2024, he served as an Active-Duty physical therapist, with his final duty station at Fort Cavazos, TX. His military training brought him to Fort Sam Houston, where he fell in love with the city of San Antonio and decided to make it his home. Throughout his career, Dr. Branch has worked extensively in outpatient settings, treating musculoskeletal injuries, orthopedic conditions, neurologic and balance disorders, vestibular conditions, and sports injuries. He particularly values serving those who serve, including military personnel and law enforcement. Branch believes that the foundation of effective healthcare is genuine love and care, creating an environment that fosters healing for patients and team members alike.
Outside of his professional life, Dr. Branch enjoys traveling, spending time at the beach, and walking in nature with his fiancé and their pup. He cherishes quality time with his three beautiful children on the weekends. A fitness enthusiast, Dr. Branch can often be found at the gym or running on a trail, embodying the active lifestyle he encourages in his patients.

Dr. Alexandra Garcia is a Wellness Center Family Medicine Physician serving our FUND patients. She believes that taking care of your health maintenance should be as routine as keeping up with your vehicle so it can keep you moving on your journey. Your health care is based on shared decision making.
Dr. Garcia has 20 years experience in Family Medicine. She earned her undergraduate degree at the University of Texas at Austin. She earned her doctorate in medicine at the University of Texas Health Science Center at Houston Medical School, now known as the John P. and Katherine G. McGovern Medical School. She completed her internship and residency with the Department of Family and Community Medicine at the University of Texas Health Science Center at San Antonio.
She is married, the mother of a son and a pug. She enjoys cooking and exploring other cultures through food, and also teaching her son to cook, She and her son are working on learning how to plant a vegetable garden.

MICHELE PHILLIPS, MD

As a family physician, I believe that continuity of care is key to fostering trust and rapport. My approach centers on listening, building relationships, understanding needs, maintaining open communication, showing compassion, and educating patients to support healthier lives.
Dr. Phillips began her career in healthcare as a nurse. Originally from Ohio, she dedicated 13 years to patient care before pursuing her medical education at Case Western Reserve University School of Medicine. Following medical school, she completed her Family Medicine residency in Akron, Ohio, and achieved Board Certification in Family Medicine. Originally from Ohio, her passion for holistic patient care has been a cornerstone of her professional journey.
After serving on active duty in the United States Air Force, Dr. Phillips joined Health by Design in 2008, bringing her passion for prevention and patient wellness. She later served as Assistant Medical Director from 2011 to 2018, where she played a pivotal role in shaping the organization’s approach to healthcare. Since returning to full-time primary care in 2018, she has focused on whole person care and chronic disease management, two areas she is deeply passionate about.
Outside of her professional life, Dr. Phillips enjoys spending quality time with her family, exploring the outdoors, and embarking on travel adventures. Her active lifestyle reflects her commitment to wellness both personally and professionally.

JONATHAN SAMS, PT, DPT

My goal is to empower patients to achieve their best movement, strength, and performance through evidence-based care and personalized treatment plans. I prioritize building collaborative relationships founded on trust, education, and active participation, enabling individuals to recover, prevent injuries, and excel in their physical pursuits. I am passionate about lifelong learning and dedicated to enhancing the quality of life for every patient I have the privilege to serve.
Dr. Sams earned his Bachelor’s Degree in Biology and Exercise Science from Pfeiffer University, followed by a Master’s Degree in Physical Therapy from East Carolina University. He later completed his Doctoral Degree from A.T. Still University. Dr. Sams is also a Board-Certified Clinical Specialist in Sports and Orthopedics, showcasing his dedication to continuous learning and professional excellence.
Dr. Sams began his career as a young physical therapist with the United States Air Force, which brought him to San Antonio, TX. After being stationed there twice, he chose to make San Antonio his home and enter private practice. He has extensive experience in outpatient orthopedic and sports physical therapy. He is also an Associate Professor at Bowling Green State University, contributing to their Doctoral Physical Therapy Program. His patient-centered approach blends evidence-based practice with individualized treatment plans to ensure optimal recovery, injury prevention, and performance enhancement.
Outside of work, Dr. Sams enjoys spending time with his wife Brady and their three children: JJ, Cameron, and baby Emma Kate. He is passionate about cooking for family and friends, playing church league basketball at Community Bible Church, and staying active at Orange Theory Fitness. He is a dedicated fan of the Dallas Cowboys and Duke basketball. He also finds joy in caring for his Blue Heeler, Hunter, and his outdoor cat, Calli.

BRIAN SO, DO

I believe that knowledge is power, and by understanding your medical conditions you are on the first step to recovery. As a Primary Care Sports Medicine physician, my goal is to help my patients who are sidelined by injuries to get back on their feet and enjoy what they do to ensure a happier and healthier life.
Dr. Brian So attended medical school at the University of North Texas Health Science Center, where he earned a degree as Doctor of Osteopathic Medicine. He moved to San Antonio in 2018 to complete his Family and Community Medicine residency at the University of Texas Health Science Center San Antonio from 2019 to 2022. Following residency, he pursued additional fellowship training in Primary Care Sports Medicine at the Texas Institute of Graduate Medical Education and Research (TIGMER), an affiliate of Sports Medicine Associates San Antonio, completing the program in 2023.
Dr. So is a board-certified Family Medicine and Primary Care Sports Medicine physician. He began his career at the FUND Schertz Clinic as a Family Medicine physician and now serves as the Sports Medicine/Musculoskeletal physician for all FUND clinics. He is passionate about helping patients achieve functionality, enabling them to perform any task they need to do, whether it involves activities of daily living or playing pickleball. Dr. So collaborates closely with his patients to understand and meet their goals.
Dr. So enjoys spending quality time with his family and exploring new places to eat in San Antonio. He and his wife, Christine Song, have two dogs, Hope and Remus. In his free time, he loves learning new things, playing basketball, and honing his culinary skills.

MAXINE WILBORN, PHD, LPC

I am a co-pilot for my clients, blending their unique strengths with practical tools to navigate life’s challenges and achieve a balanced, healthy mental health lifestyle, always prioritizing their wants and needs through patient-centered care.
Dr. Wilborn received her BS in Psychology from The University of Maryland, along with a Certificate in Clinical Mental Healthcare and a minor in Gerontology. She earned her MS in Clinical Mental Health Counseling at Walden University and her PhD in Counseling Education and Supervision, also at Walden University. Her training includes behavior analysis, trauma, Acceptance and Commitment Therapy, and Mindfulness, all aimed at promoting overall well-being and self-care.
Dr. Wilborn has worked in private practice, helping individuals, couples, and families develop skills they can use to navigate both current and future life events. She specializes in working with individuals, families, and couples facing challenges such as anxiety, depression, crisis, self-esteem issues, trauma, abuse, and relationship concerns. Her practice focuses on utilizing evidence-based practices, including Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), person-centered approaches, mindfulness, and behavior analysis, to support her clients' mental health and well-being.
A native Texan raised in San Antonio, Dr. Wilborn enjoys spending time outdoors camping, fishing, and embarking on moto trips. In her free time, she loves gardening, baking, reading professional journals, and spending quality time with her family. Her family includes her husband Zachary, their daughter Kacie, and two grandchildren, Kameryn and Jake. The family wouldn’t be complete without their 4-year-old Wire-Haired Pointing Griffon, Stella, and Tails, their orange barn cat.
